RISE & DREAM Foundation provides Thanksgiving and holiday assistance to families experiencing financial hardship. Assistance is made possible through the generosity of donors and community partners and is based on available resources.

Submitting this form is an application for consideration and a request to be sponsored. It does not guarantee that your family will be selected or receive assistance.

*I understand that if my household is approved for RISE & DREAM Foundation holiday assistance, I cannot simultaneously receive duplicate holiday assistance for the same household/children through multiple holiday sponsorship programs. If approved by more than one program, I agree to choose which program my household will participate in and notify the other organization.
